- FHW Training Course From Government Recognized Organization OR
- ANM From Indian Nursing Council Or Candidates Should be must Registered in Gujarat Nursing council.

- 35 Years.
- Age Relaxation as per government rules
- General Category Female : 5 Years
- Reserve Category Male : 5 Years
- Reserve Category Female : 5 Years
- PHW General Category Male : 5 Years
- PHW General Category Female : 10 Years
- PHW Reserve Category Male : 10 Years
- PHW Reserve Category Female : 15 Years
